Community RCME Model

Introduction

The Joint Standing Committee’s Community RCME Program will provide annual funding for collective learning. To start the process, please use this questionnaire to describe the structures and processes your community will use to plan, develop and deliver community RCME. 

After you complete this survey, one of our implementation team members will share your responses with you in the form of a Draft Model for Community RCME.  We will then work with you and other physicians in your community to develop an optimal model that responds to your local situation.  This may include improvements to your current practices or collaborating with other communities to create a model that includes several communities.

There is no requirement to complete the questionnaire.  If you would prefer to discuss your community’s vision for community RCME, you can contact us at CommunityRCME@RCCbc.ca and we can help by phone or videoconference.
