Proof of Citizenship

To verify your eligibility for this program, please submit the following documents:

  • Proof of Citizenship/Immigration Status: Documentation confirming you have Canadian citizenship, Canadian permanent resident status, or appropriate authorization under the Immigration and Refugee Protection Act (Canada) that permits you to engage in the practice of nursing in Ontario.
  • Official Transcripts: Transcripts showing a minimum 65% cumulative average from an Ontario PSW program that meets the Ontario Ministry of Colleges and Universities program standards (including programs offered by public colleges, private career colleges, or district school boards in Ontario).
  • Letter from Employer: A letter confirming you have worked as a PSW in Ontario within the past five years on at least a part-time or casual basis.

Please ensure all documents are clear, legible, and up to date before submitting.

Examples of supporting documents for "Permanent Resident" or "Other" (Birth Certificate, PR Card, Notice of Decision letter, NOT a driver's license).
