Suicide Prevention and Self Harm Feedback Survey

The Ministry of Children and Family Development is collecting this information under section 26(c)(e) of the Freedom of Information and Protection of Privacy Act for the purpose of gathering feedback to improve future iterations of the Suicide Prevention and Self Harm Video Series and Resources, understand its impact, and to collect email addresses for the purpose of a prize draw.

Responses are encouraged but not required.  Please do not include any personally identifiable information about yourself or others in your responses. Participation in this questionnaire has no effect on your ability to access the Suicide Prevention and Self Harm resource(s).

Your feedback is appreciated, and it will help inform future content and structure of these resources. This anonymous survey will take you approximately 15 minutes to complete.
