2022 AMMI Canada Lifetime Achievement Award

The purpose of the AMMI Canada Lifetime Achievement Award is to recognize an AMMI Canada member in the fields of infectious disease and/or medical microbiology who has made outstanding contributions in any of or all the following areas: clinical care, research, education, administration, or advocacy. Recipients of the AMMI Canada Lifetime Achievement Award will be granted the designation of FAMMI (Fellow of AMMI Canada) and receive a monetary award of $1,000 CDN.

AMMI Canada is committed to the principle of equity and the award will be given on the basis of merit.  We welcome diversity and encourage applicants from all qualified persons, including persons of all gender identities, persons with disabilities, members of visible minorities, and Indigenous persons.  Recognizing that applicants from underrepresented groups may be focused on producing work that meets community needs or developing policies, this will also be recognized and equally valued and should be outlined in the letter of intent.

Eligibility of Nominee
  1. Current or past Active membership in AMMI Canada for a minimum of seven consecutive years (excluding leaves). The award may be bestowed posthumously.
Application Requirements 
  1. Letter of Intent. The letter must include a brief narrative summary of the outstanding contributions of the nominee. Nominations of others (with their consent) and self-nominations are acceptable.
  2. Nominee’s CV (Canadian Common CV or Academic CV are acceptable formats).
  3. Two Nominators (one from within and one from outside the nominee’s institution). Both nominators MUST be Active members of the AMMI Canada in good standing.
  4. Two Letters of Support, one from each of the nominators outlining why the nominee should be considered for the Lifetime Achievement Award using the narrative summary.
  5. A narrative summary of the nominee’s career accomplishments, which will be used at the awards ceremony and in publications. NOTE: This summary is required for the nominee to be considered for the award (1-2 pages). The summary should include evidence of the following contributions (as relevant):
  • A brief overview of the nominee’s career.
  • Major contributions to the acquisition and dissemination of knowledge in the fields of infectious disease and/or medical microbiology or public health.
  • Contributions to clinical practice in infectious diseases.
  • Contributions (direct or indirect) to patient care that have significantly advanced the practice of infectious disease and/or medical microbiology.
  • Evidence of excellence in mentoring and/or teaching of fellows, residents, or medical students.
  • Advocacy that has resulted in improved patient care.
  • Other relevant contributions.
Review and Selection Process
All nominations will be reviewed by the AMMI Canada Council.
 
Nominators and winners will be notified of the Council’s decision in early March 2023. If no nominations are received or meet the criteria, no award will be granted for that year.
 
Announcement of the Award
The 2022 AMMI Canada Distinguished Service Award will be presented at the 2023 AMMI Canada – CACMID Annual Conference.
