*First Name:

*Last Name:

*Current age and grade:

*Address:

Participant's email (if you have one):

*Parent/guardian's email:

*Parent/guardian's phone number:

*Are you able to attend the virtual sessions on March 22 and 25?

In this program, illustrators will create pieces of art inspired by the theme of "Nature as Character." Do you have an idea of what natural element you would like to focus on (tree, flower, sun, water, etc), and what form of art you would like to create (drawing, digital art, photo, etc)?

*Can you tell us why you are excited to be part of Tell Your Story?

*You will have to work on your project at home, in between our virtual meetups (ex: making rough copies, making good copies, uploading to a computer, etc.). Can you commit to doing this?

*I understand that, if I am selected as an illustrator for Tell Your Story, there will be deadlines that I need to meet. If I am not able to complete my work on my project in time, it may not be published as part of the ebook anthology.

*We will need permission from your parent or guardian to make your art available in the Library collection as part of the Tell Your Story ebook. Does your parent/guardian know that you are applying for this program, and that we will be contacting them for permission during the program?

We will be selecting artists to participate in this Tell Your Story program and will contact you by email (participant and parent’s email) to let you know if you have been selected.

If you’re not selected this time, there will be other Tell Your Story projects in the future.
