Thank you for your interest in the CSHP Excellence in Hospital Pharmacy Program. The Excellence program is entering into an exciting new chapter and we need your help!

Recently, the Excellence program launched the targets for Excellence, and is currently restructuring its composition. We are looking for volunteers on many levels. By completing this survey, the Excellence program will build its pool of volunteers. Your information will be kept for future reference. As plans are made, we will reach out to our volunteers for assistance.
