Youth Survey: Experiences Accessing Health Services

This survey is for young people aged 12 to 24 who have, or may be at risk of having, mental health and substance use challenges. We are interested in your experiences accessing services in order to improve young people's access to needed services in the community.

This survey takes around 15 minutes to complete. It is being distributed by McCreary Centre Society in partnership with The Foundry.

This survey is confidential and anonymous.
Nobody will be able to connect your answers to you personally.

This survey is voluntary.
You can stop taking part at any time and can skip questions you don't feel comfortable answering.

Once you complete & submit your survey, you can choose to enter your name in a draw to win one of ten $30 gift cards (of your choice). Instructions on how to enter the draw will appear on the screen after you submit your survey. If you choose to enter the draw, we still won't be able to link your name to your survey answers -- your survey will remain anonymous.
