SURE Ongoing Feedback Survey

We invite you to participate in this short survey on the Canadian Substance Use Resource and Knowledge Exchange Centre (SURE). In requesting feedback on the site, we are interested in hearing about your experience with site navigation, functionality, and content, as well as any ideas you may have about how we can improve the site in the future. 

Your participation is voluntary and by completing the questionnaire you consent to the collection of information. Information gathered in this survey will be used by the Canadian Public Health Association to improve SURE and related knowledge exchange activities, and for internal reporting purposes. Your information will remain confidential and any information you give that might identify you or your organization will be removed prior to sharing any results. Your responses will be stored on CPHA servers and access to this data will be limited to CPHA staff.
