Managed Forest Tax Incentive Program
MFTIP Ten-Year Application
This application form must be submitted by your Managed Forest Plan Approver, along with an approved Managed Forest Plan for your property(ies) and a current Property Assessment Notice, to apply for the MFTIP for the next tax year. The deadline to apply is June 30th for properties new to the program, July 31st for renewals, and 90 days after the date of transfer for ownership changes. Once you have completed this form, you can download a copy. If you provide an email address, a copy will be emailed to you.
This application is prepared and submitted under Section 8.2 to 9.7 of Ontario Regulation 282/98 of the
Assessment Act
.
Note: All landowners participating in a plan must sign an application form. Up to three landowners can sign one form. Additional landowners should complete a separate form.

2. Type of Application
Select 'New plan' if the property has not previously participated in the MFTIP.
Select 'Ten-year Renewal Application' if you are renewing an existing MFTIP plan which is still in effect. If you missed your renewal deadline, and are re-applying, select 'New plan'.
Select 'Purchaser Applying to Continue in the MFTIP with a New Plan' if you are a new owner submitting a plan for a property which is currently enrolled in the MFTIP. If you have recently purchased a property but it is not currently enrolled in the MFTIP, select 'New Plan'.

3. For the Property being Entered into the MFTIP, I certify that:
This land is owned by,
i. an individual who is a Canadian Citizen or has been lawfully admitted for permanent residence,
ii. two or more individuals as joint tenants or tenants in common if 50 per cent or more of the beneficial interest in the land is held by persons described in i above,
iii. a corporation that has issued and allocated shares to which are attached more than 50 per cent of the voting rights ordinarily exercisable at meetings of the shareholders and that are owned by individuals described in i above,
iv. a partnership of which more than 50 per cent of the income or loss of the partnership is allocated to partners who are persons described in i or ii above,
v. a conservation authority, or
vi. a trust established by a person described in subparagraph i or ii, a partnership described in iii above or a conservation authority, but only is 50 per cent or omre of the beneficial interest in the trust property is held by those persons, partnerships or conservation authorities.
Approved Managed Forest Plan:
The property in this application will be managed according to the Approved Managed Forest Plan that is referred to in this application. Activities on this property will be in accordance to "good forestry practices" as defined in the
Forestry Act
. If I do not managed according to program standards, the property will be deemed to have been ineligible for the program.
Ten-Year Application:
If I decide to continue in the program after this ten-year application has expired, a new ten-year application will be submitted. If I decide not to reapply, I will submit a Landowner Report and a copy of the Report of Activities (Section 9) from my plan. If I do not submit this information, the property will be deemed to have been ineligible for the program.
Five-Year Progress Report:
By July 31 of my fifth year in the MFTIP, I will submit a Five-Year Progress Report, a copy of the Report of Activities (Section 9) from my plan and any changes to the Ten-Year Activity Summary (updates to Section 8) from my plan. If I do not submit this information, the property will be deemed to have been ineligible for the program.
Ineligible properties that have received program benefits:
I am aware that if the Ministry of Northern Development, Mines, Natural Resources and Forestry finds that a property was not eligible for the MFTIP while receiving program benefits, NDMNRF may remove the property from the program for the current year and up to four prior years. The municipality may then collect property taxes equal to the difference between taxes collected while the property was in the MFTIP and those that would have been collected had the property not been in the program during those years.
Right to Inspect:
The Ministry of Northern Development, Mines, Natural Resources and Forestry (and their agents) may enter onto the property listed in this application, while the property is in and up to four years after the property has been removed from the Managed Forests property class, to confirm that the land is/was eligible for the MFTIP.
Freedom of Information:
The Ministry of Northern Development, Mines, Natural Resources and Forestry is collecting information in this application, under the authority of Section 9 of the Ontario Regulation 282/98 of the
Assessment Act
, for the purposes of administering the MFTIP including communications with you and the Municipal Property Assessment Corporation. Any information that you submit as part of this application may be shared between other NDMNRF programs. Any personal information you provide will be protected in accordance with the
Freedom of Information and Protection of Privacy Act
