EASE 8-12 Course Feedback Survey

The Ministry of Children and Family Development is collecting this information under section 26(c)(e) of the Freedom of Information and Protection of Privacy Act for the purpose of gathering feedback to improve Everyday Anxiety Strategies for Educators (EASE) and understand its impact, and for the purpose of collecting emails to enter a prize draw. Prize draw has closed as of June 29, 2022.

Please note this questionnaire is voluntary, responses are encouraged but not required. Please do not include any personally identifiable information about yourself or others in your responses. Your responses are anonymous. Participation in this questionnaire has no effect on your ability to access EASE.

Your feedback is appreciated and it will help inform future content and structure of this course. This anonymous survey will take you approximately 15 minutes to complete.  Once you have completed the survey, you will have the option to be redirected to enter your name for a draw for one of three $100 bookstore gift cards.
