Approximately 22% of adults in Canada experience high blood pressure (hypertension). Effective management of your blood pressure can reduce the risk of heart disease and strokes.
White Rock-South Surrey residents are invited to join a free education session on how to effectively manage high blood pressure. Walk away with valuable insights, tips and practice skills for implementing lifestyle changes that can lower your risk. Session Format: Virutal group session on the first Friday of every month from from 12:00 p.m. to 1:00 p.m. The sessions are offered online via Zoom. Space is limited to 20 participants. You are invited to take part in this survey. We are asking people who are registering in the Blood Pressure education session. We are doing this survey to learn more about the demographics of those attending the sessions, and ways we can improve future sessions for patients. The personal information we collect is protected under the BC Freedom of Information and Protection of Privacy Act (FIPPA). We ask you to allow us to collect, use, and keep your responses because our questions ask for your demographic/contact information, personal views and opinions. Your responses are considered personal information.
Who sees my responses?
Only the survey administrator, White Rock South Surrey Primary Care Network, will see your responses.
How will my answers be used?
We will run reports based on the responses to the survey. We are using the results from this survey to collect information the demographics of individuals who are attending the education session. We will share the survey results with the Primary Care Network and Division of Family Practice. We will not release or use your information for any other reason, unless permitted by law.
Your information will be entered into Simple Survey, a company in Canada, and is subject to Canada law. Simple Survey protects your responses under their Privacy Policy and Terms of Use. Your responses will be retained on the Simple Survey platform for 12 months and will be deleted by the survey administrator after that time.
It is your choice
You can choose to take part and complete the survey or not. You can stop the survey at any time. You can answer the questions you want to and skip the ones you do not want to answer. If at any time you want us to delete your answers, please contact the survey administrator.
No matter what you decide or what you tell us, it will not affect the care you receive at Fraser Health now or in the future.
