Registration - Lifestyle Management Series

Take charge of your health with the free Lifestyle Management Education Series! White Rock-South Surrey residents are invited to join this four-part series designed to equip you with the knowledge and skills to make meaningful lifestyle changes. Through four interactive 90-minute online sessions, you’ll set goals, develop strategies to overcome barriers, and put what you learn into action—one step at a time. Below are the topics covered:

  1. Managing Lifestyle Change: Review the differences between behaviour change and lifestyle change, along with exploring how to create, implement, and achieve small sustainable goals that can impact health.
  2. Movement for Health: Learn the importance of being physically active, how it differs from exercise, and ways to incorporate physical activity into daily life. SMART goals related to physical activity will also be set.
  3. Food and Nutrition: Review the different aspects of healthy eating, the foundations of nutrition, making healthier choices, and understanding nutrition labels. SMART goals related to healthy eating will also be set.
  4. Sleep: Learn about the importance of sleep, how sleep affects health, the stages of sleep, and tips for getting a good night’s sleep.

Session Format: 4-part virtual group series includes 90-minute sessions held once a week for 4 weeks. Participants must attend all 4 sessions. The sessions are offered online via Microsoft Teams. Space is limited to 10 participants.

You are invited to take part in this survey. We are asking people who are registering in the Blood Pressure education session.  We are doing this survey to learn more about the demographics of those attending the sessions, and ways we can improve future sessions for patients. The personal information we collect is protected under the BC Freedom of Information and Protection of Privacy Act (FIPPA). We ask you to allow us to collect, use, and keep your responses because our questions ask for your: Name, Date of Birth, Personal Health Number (PHN), your personal demographics, contact information, and personal views and opinions. Your responses are considered personal information. 

Who sees my responses?

Only the survey administrator, White Rock South Surrey Primary Care Network, will see your responses.

How will my answers be used?

We will run reports based on the responses to the survey. We are using the results from this survey to collect information the demographics of individuals who are attending the education session. We will share the survey results with the Primary Care Network and Division of Family Practice. We will not release or use your information for any other reason, unless permitted by law.

Your information will be entered into Simple Survey, a company in Canada, and is subject to Canada law. Simple Survey protects your responses under their Privacy Policy and Terms of Use. Your responses will be retained on the Simple Survey platform for 12 months and will be deleted by the survey administrator after that time.

It is your choice

You can choose to take part and complete the survey or not. You can stop the survey at any time. You can answer the questions you want to and skip the ones you do not want to answer. If at any time you want us to delete your answers, please contact the survey administrator.

No matter what you decide or what you tell us, it will not affect the care you receive at Fraser Health now or in the future.
