The Collective at MTN Village Application Form

Before You Apply

Please review the eligibility requirements on the The Collective at MTN Village page before starting your application.

Important

• Applications are reviewed on a first-come, first-served basis once all required documents are submitted.
• Only one application per household may be submitted.
• Incomplete applications will not be reviewed.

This is a two-step process

1. Housing Squamish reviews applications for eligibility.
2. Eligible applicants are referred to Anthem for tenant selection.

Meeting eligibility requirements does not guarantee tenancy.

Documents Required

All household members aged 19 and over must provide:

• 2025 T1 General Tax Return
• 2025 Notice of Assessment

Additional documents are required. See below. 

• Employed applicants: 3 most recent pay stubs.
• Business owners: District of Squamish business licence + proof of work hours.
• Self-employed applicants: proof of hours worked + that a portion of income comes from clients in the District of Squamish.
• Retired or unable to work due to disability: proof of pension, benefits, or income support + that you lived or worked in the District of Squamish previously.

📌 Applications are not considered complete until all supporting documents are received.

📌 We highly recommend gathering your documents before starting. 

📌 All files should be uploaded as PDFs.  

Information You’ll Need During the Application

• Employer contact information for the past year
• Address history for the past year
• Driver’s licence number
• Landlord references (only contacted after eligibility is confirmed and you choose to proceed with a lease)
